White miso vs. red miso: what's actually different?
Three levers, really: how much koji relative to soybeans, how much salt, and how long it sits.
Sweet white miso (shiro) uses a high proportion of koji, less salt, and a short, warmer fermentation — sometimes only a few weeks. All that koji-derived sugar plus minimal aging gives you a pale, mild, faintly sweet paste. Red miso (aka) inverts it: more soybean, more salt, and a long maturation, commonly six to twelve months, during which the Maillard reaction — non-enzymatic browning between amino compounds and sugars — darkens the paste and builds the aroma compounds that make it taste so much more intense [1].

A 2023 review in Foods describes this conversion in detail and notes that pharmacokinetic studies have generally shown faster and higher absorption after ingesting aglycones than glucosides — while adding, fairly, that other studies have found the opposite and "a consensus is yet to be established" [3]. The same review makes a point worth holding onto: conversion efficiency "varies greatly between species but mainly between strains." Two miso pastes fermented by different microbial communities are not interchangeable at the molecular level.
Laboratory work supports the direction of travel. In one 2023 study, soymilk given a combined enzymatic and fermentation treatment ended up with daidzein and genistein roughly 18-fold higher than untreated control [4] — though, as the authors note, that was simulated digestion in a lab using concentrated extracts, not absorption measured in people eating food.
Longer aging means more time for all of this, plus more protein broken into free amino acids and more Maillard browning. Whether a year-old red miso is meaningfully "better" for you than a six-week white miso is not a question the research has answered. A 2024 review of A. oryzae found only a handful of published studies examining it in a postbiotic context, none of them in humans, and noted that only about 1.7% of the mold's roughly 12,000 genes have been functionally verified [2]. What about the salt in miso?
This is the question I get most, and it deserves a straight answer: yes, miso is salty. Red miso more so than white. A typical bowl of miso soup carries somewhere in the range of 1–2 grams of salt.
There's an interesting thread of research here, and it needs careful handling. A 2020 review in Environmental Health and Preventive Medicine gathered the studies on habitual miso soup consumption and reported an absence of a relationship between how often people ate miso soup and their blood pressure levels, with the most consistent observation across studies being a lower heart rate in frequent consumers [5]. That's interesting. It is also, as the review's own author states, not enough: the paper concludes plainly that the data are inadequate to draw conclusions about miso's effects on blood pressure and heart rate in humans, and that interventional studies are needed. People who eat a lot of miso soup tend to eat a traditional Japanese diet generally, and observational studies show associations, not cause and effect.
